What vitamin B12 does
Vitamin B12 is an essential nutrient needed to make healthy red blood cells and to keep the nervous system working properly. Because the body cannot make it, B12 must come from the diet — mainly animal products — or supplements. A blood B12 test is the usual first step in checking B12 status, often done when someone has tiredness, nerve-related symptoms such as pins and needles, or a type of anaemia. Why B12 results need careful interpretation
The total B12 measured in a standard blood test does not always reflect how much active B12 is available to your cells, which is why borderline results (roughly 200–300 ng/L) can be genuinely uncertain. Someone with a low-normal level but clear symptoms may still be deficient, while a normal level usually reassures. Because of this, doctors interpret B12 alongside symptoms and, where needed, additional tests such as active B12 (holotranscobalamin) or methylmalonic acid, rather than treating one number as definitive.
Low B12 and its causes
A low B12 can cause tiredness, weakness, pins and needles, balance or memory problems, mouth soreness and, over time, a form of anaemia. Importantly, deficiency is often due to a problem absorbing B12 — such as pernicious anaemia, an autoimmune condition — rather than simply too little in the diet, though vegans and strict vegetarians who do not supplement are also at risk.
