What ferritin measures

Ferritin is a protein that stores iron in the body, and a blood ferritin test is the most commonly used way to assess how much iron you have in reserve. Low ferritin is one of the earliest and most specific signs of iron deficiency, often falling before haemoglobin drops and before anaemia develops. This is why ferritin is central to investigating tiredness and suspected iron deficiency. This tool shows where a ferritin value sits within general reference ranges, for education only.

Why ferritin must be interpreted in context

Ferritin is an acute-phase reactant, meaning it rises with inflammation, infection, liver conditions and some other situations independently of iron stores. So a normal or high ferritin does not always rule out iron deficiency if inflammation is present, and a high ferritin does not automatically mean iron overload. Doctors interpret ferritin alongside other iron studies such as transferrin saturation, symptoms and clinical context, rather than in isolation.

Low ferritin and iron deficiency

A low ferritin is the hallmark of depleted iron stores and can cause tiredness, weakness and shortness of breath even before full anaemia develops. Identifying why ferritin is low is an important part of treatment — common causes include blood loss including heavy periods, low dietary iron, increased needs in pregnancy, and absorption problems. Frequently Asked Questions

Reference ranges vary by laboratory and sex, but broadly a ferritin below about 30 µg/L strongly suggests low iron stores, while upper limits are often around 200 µg/L for women and 400 µg/L for men. Your own report's range and your doctor's interpretation are what matter, as ferritin is affected by inflammation and other factors.
